Predictive Factors of Mortality during Acute Generalized Peritonitis due to Intestinal Perforation of Typhoid Origin in Mbujimayi, DR Congo

Abstract:
Objective
Determine the frequency and predictive factors of mortality during acute generalized peritonitis due to intestinal perforation of typhoid origin in Mbujimayi.
Methods
This was a prospective and analytical study conducted in two hospitals in the city of Mbujimayi, Democratic Republic of Congo (DRC). Our sample consisted of 86 patients who underwent surgery for acute generalized peritonitis due to intestinal perforation of typhoid origin.
Results
The frequency of acute generalized peritonitis due to intestinal perforation of typhoid origin was 9.4% among all surgical emergencies. We noted a male predominance (sex ratio = 1.2). The mean age of patients was 19.0 ± 17.0 years. The Mannheim Peritonitis Index (MPI) was high (≥30) in 54.7% of patients and the overall mortality was 39.5%. Apart from moderate/high MPI score (p = 0.016), low socioeconomic level (p = 0.026), location of perforation < 15cm of the ileocecal valve (p = 0.045) and the presence of postoperative complications (p = 0.000) were found to be predictive factors of mortality in patients suffering from acute generalized peritonitis due to intestinal perforation of typhoid origin in Mbujimayi.
Conclusion
Acute generalized peritonitis due to intestinal perforation of typhoid origin is a significant and common surgical emergency, associated with a high mortality rate in Mbujimayi. The predictive factors for this mortality are identified in this study.
