Abstract:

This paper provides a comprehensive review of existing literature evaluating the impact of pharmacists' involvement in chronic disease management on patient outcomes and healthcare efficiency. The review explores various chronic diseases, including diabetes, hypertension, asthma, and others, examining the pharmacist's role in medication adherence, clinical outcomes, healthcare utilization, and cost-effectiveness. Additionally, the paper discusses barriers and facilitators to pharmacist-led interventions and identifies areas for future research and practice development in optimizing pharmacists' role in chronic disease management. Chronic diseases pose a significant burden on healthcare systems worldwide, requiring comprehensive management strategies to optimize patient outcomes and healthcare efficiency. Pharmacists play a crucial role in chronic disease management through medication management, patient education, and collaborative care.
