Abstract:

Chronic diseases represent a significant challenge to healthcare systems worldwide, necessitating innovative approaches to management that enhance patient outcomes. This review proposes a conceptual framework for integrating health data analytics into chronic disease management, highlighting its potential to improve early detection, facilitate timely interventions, and enable personalized care. The framework emphasizes essential components such as data collection, analysis, integration, and feedback, leveraging diverse data sources including electronic health records and wearable technologies. The proposed framework aims to bridge existing gaps in current chronic disease management systems by fostering patient engagement and encouraging active participation in health management. Furthermore, the integration of health data analytics promotes collaboration among healthcare providers, patients, and technologists, facilitating dynamic care plans tailored to individual needs. This review outlines the implications of health data analytics for enhancing chronic disease management and provides recommendations for healthcare stakeholders to optimize the benefits of this transformative approach.
