Cloud Interoperability Model for Secure Health Information Exchange across Multi Organization Systems

Abstract:
Secure and seamless exchange of health information across multiple organizations remains a persistent challenge due to heterogeneous systems, varying data standards, regulatory constraints, and escalating cybersecurity threats. This study proposes a Cloud Interoperability Model for Secure Health Information Exchange (CIM-SHIE) designed to enable trusted, scalable, and standards-driven interoperability among multi-organization healthcare systems. The model integrates cloud-native architecture with interoperability standards, security-by-design principles, and governance mechanisms to support cross-institutional data sharing without compromising privacy, integrity, or availability. CIM-SHIE is structured around four interoperable layers: infrastructure, data, service, and governance. At the infrastructure layer, secure hybrid and multi-cloud deployments provide elasticity and resilience. The data layer leverages standardized health data formats and semantic harmonization to ensure syntactic and semantic interoperability across disparate electronic health record systems. The service layer employs application programming interfaces and microservices to enable modular, real-time health information exchange. The governance layer incorporates access control, identity management, auditability, and compliance monitoring aligned with prevailing health data protection regulations. Advanced security mechanisms, including encryption, role-based and attribute-based access control, and continuous monitoring, are embedded across all layers to mitigate risks such as unauthorized access, data leakage, and interoperability-induced vulnerabilities. The model also emphasizes trust frameworks and policy alignment among participating organizations, enabling collaborative data sharing while preserving institutional autonomy. A conceptual evaluation demonstrates that CIM-SHIE improves interoperability maturity, reduces integration complexity, enhances data security, and supports scalable collaboration across healthcare providers, laboratories, insurers, and public health agencies. By addressing technical, organizational, and security dimensions simultaneously, the proposed model provides a holistic framework for secure cloud-enabled health information exchange. This study contributes to health informatics research by offering a structured, adaptable interoperability model capable of supporting digital health transformation, population health management, and data-driven clinical decision-making in complex, multi-organization healthcare ecosystems.
