Abstract:
Effective healthcare risk management depends on timely, accurate, and outcome-oriented reporting mechanisms. Traditional reporting frameworks often fail to close the feedback loop between incident tracking and organizational learning, thereby impeding safety improvement and strategic response in clinical settings. This paper proposes a comprehensive outcome-oriented reporting framework specifically designed to enhance healthcare risk management and incident tracking across institutional settings. Drawing from a synthesis of literature in patient safety, clinical governance, and health informatics, this study develops a conceptual model that emphasizes standardized data collection, real-time analytics, and feedback integration to drive quality improvements. The proposed framework integrates both retrospective incident reporting and prospective risk assessment tools, facilitating proactive organizational learning. The study is based on a systematic literature review methodology with a focus on multi-institutional implementations, legal compliance, and interoperability. By aligning risk data with measurable clinical outcomes, this framework aims to strengthen safety culture, support regulatory compliance, and enhance evidence-based decision-making in healthcare institutions.
